A sliding fee scale is available for those wishing or needing to assume full costs for services. Please direct inquiries regarding private pay to [email protected] or by calling 920-723-0307. If you are uninsured or opting to not submit claims with  an insurance plan or coverage, you are entitled to a Good Faith Estimate of expected charges. 

Disclaimer for Good Faith Estimate:

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage, or a Federal health care program, or not seeking to submit claims with an insurance plan or coverage, the costs associated in their care, both orally and in writing, by providing a Good Faith Estimate (GFE). 

The GFE shows the costs of items and services that are reasonably expected for services provided by Thrive Psychology, LLC. The estimate is based on information known at the time the estimate was created. 

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider , and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. You may contact Sheila Kozler to let her know the billed charges are higher than the Good Faith Estimate. You can ask her to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available. 

Make sure to save a copy or picture of your Good Faith Estimate.

Services may be covered in full or in part by the above listed or other health insurance/employee benefit plans. Please check your coverage carefully by asking the following questions:

    • What are my behavioral health benefits?
    • Do I have a deductible and has it been met?
    • Is the therapist I wish to establish care with "in-network" with my insurance carrier? 
    • Do I have a co-pay for each office visit? How much is my co-pay?
    • How many therapy sessions does my plan cover per covered period? When does that period renew?
    • How much does my insurance pay for an out-of-network provider?
    • Is a referral required from my primary care physician prior to my first appointment with a therapist? 

Reduced Fee 
Reduced fee services are available on a limited basis. 

Cash, check, and all major credit cards are accepted for payment (through secure client portal).

Cancellation Policy 
If you know that you will be unable to attend your upcoming appointment, please call as soon as possible to reschedule. A 24-hour notice allows someone else to use the newly opened appointment time. However, it is understandable that unexpected or uncontrollable events occur (e.g., bad weather, sick children, technological difficulties when using tele-health) causing late cancellations. If late cancellations or missed appointments occur due to a controllable event (e.g., forgot, double-booked) then a $50.00 missed appointment fee will be charged.
