If you are a new client, once I have your permission to use an authorized email and phone number, you will be sent a link to access your secure client portal through Simple Practice, where you will find all relevant forms. These forms include information on practice policy, informed consent for therapy, informed consent for tele-health services (if applicable), informed consent for in--person services during COVID-19 Public Health Crisis, insurance information, and an intake/assessment questionnaire.
In addition, if you would like me to coordinate care with another provider (e.g., primary care physician), individual (e.g., spouse) or institution (e.g., school, employer), you will find the appropriate form in the client portal.
